Background To investigate the association of maternal pre-pregnancy body mass index (BMI) and gestational weight gain (GWG) with childhood overweight and adiposity in preschool children. Methods A total of 4303 preschool children aged 3-5years were enrolled in our study. Children were defined as overweight and obesity according the criteria of World Health Organization (WHO) and maternal pre-pregnancy body status were using China adult reference. Gestational weight gain was defined according to the Institute of Medicine (IOM) guidelines. Results After adjusting the possible confounding factors, results from the logistic regression showed that maternal pre-pregnancy overweight and obesity increase the risk for childhood overweight and obesity(OR=1.82, 95%CI: 1.368-2.422). ANCOVA results also found that maternal overweight and obesity before pregnancy and excessive maternal weight gain during pregnancy increased BMI Z-score in children. Maternal GWG over the recommendation level was associated with childhood overweight and obesity (OR= 1.30 95%CI: 1.007 -1.667). Joint associations of pre-pregnancy BMI and inappropriate GWG were also noticed in the study: Stratified analysis was conducted in three groups according to the pre-pregnancy BMI of the mother, and it was found that there was no statistical difference in the risk of overweight and obesity in children (P>0.05). But when compared to mothers with adequate pre-pregnancy higher BMI and adequate GWG, combination of high pre-pregnancy BMI and excessive GWG adverse effects on childhood overweight and obesity were much higher, OR (95%CI) values was(OR=1.57, 95%CI: 1.029 - 2.409). Conclusion Both high pre-pregnancy BMI and inappropriate GWG are associated with greater offspring BMI. Pregnant women should achieve appropriate weight gain and help prevent obesity in their children.
